Older Adult Team
Our Access Team is the first contact to request mental health services, and can be reached at (707) 565-6900.
We provide outreach and engagement and outpatient specialty mental health treatment services to adults ages 60 and older. Our multidisciplinary team is able provide a wide array of services to older adults living with serious mental illness, including:
- Medication education, monitoring, and delivery
- Case management
- Referrals
- Visiting clients when hospitalized (either medically or psychiatrically) and facilitating communications between the medical and psychiatric staff for care and follow-up planning
- Transportation services, including attending important doctor’s appointments, having routine laboratory work, and participating in community-offered services to reduce isolation
We also maintain a relationship with the Bridges to Health program, which helps link clients to their primary care with much greater ease.
Our Outreach Liaison receives referrals from the Human Services Department's In-home Supportive Services and provides in-home assessment and care coordination for older adults who show symptoms of depression, serious mental illness, and/or suicidal thinking.
Funded by the Mental Health Services Act
This program receives funding through the voter-approved Mental Health Services Act (Prop. 63). MHSA funding provides a broad continuum of prevention, early intervention and services, and the necessary infrastructure, technology and training elements to effectively support our local mental health services system.
