Get the Care you Need to Remain at Home

When serious health concerns make it hard to get medical appointments, prepare meals, maintain hygiene, and manage other daily activities, living independently can be challenging.

Multipurpose Senior Services Program

The Multipurpose Senior Services Program (MSSP) Assists Sonoma County residents 65 or older who would rather live at home than in a nursing facility. Our friendly care manager snad nurses work with each client to find out what they want and need, then create a plan to help them meet their goals. When new challenges arise, the MSSP team will help figure out options and navigate concerns.

MSSP can help arrange:

In-home support services
Home-delivered meals
Transportation to medical appointments
Respite care for family caregivers
Minor home safety repairs
Medical supplies and equipment
A personal emergency response system
Counseling to support mental health

Who is eligible for MSSP?

65 years of age or older
Eligible for Medi-Cal
Needs help with daily activities such as bathing, useing the bathroom, walking or eating
Has the interest and ability to work with the care team or has a support person who can act on their behalf
